Dear patient,

When you use the practice's portal to transact business with us online and to make your file available you need to log in with your personal e-mail address + password.

In the short term, we will start using DigiD.

We do this because we keep up with national standards of data protection.

This is what it means for you:

1. Your current login details will expire.
2. To log in, you need to be registered with DigiD.

Do you have no registration with DigiD?   

Then you will need to register. This can be done by clicking on the logo below or via this link.

For your application, you will need the following: 

• citizen service number (BSN)
• address at which you are registered with your municipality
• (mobile) phone

After your application, you will receive a letter with an activation code to activate your DigiD. DigiD will send this letter within 3 working days to the address at which you are registered in your municipality.
