Patient portal

The patient portal is a modern addition we offer you as a practice. The application is available in any web browser, whether you use a PC, laptop, tablet or smartphone.

 

By logging in to the application, you as a patient can immediately arrange various matters with your GP practice, without having to be on hold on the phone. For example, you can schedule an appointment online at the specially set up time slots, order repeat medication, view blood and X-ray results including the corresponding doctor's advice and send an e-mail consultation to your GP yourself. With this, we as a practice also comply with the national requirement for electronic access to your own patient records.

 

Medical information is secure as the portal is developed to the accepted standards for handling sensitive information. Secure network systems and adapted software are used.

How the patient portal works

Below you can read which steps are necessary to create a profile. Because of security requirements, it is important that each person has their own mobile number and e-mail address. Such information can NOT can be used for multiple profiles.

A patient portal should be created by your GP practice using your personal data. To comply with all privacy laws, it is important that only you can view your medical data.

  • The e-mail address must be a personal email address be that only used by you!
  • The mobile phone number must be a personal number be that only used by you.

Complete the form below with your personal details. Within 72h, we will create your account and you will receive an e-mail from us validating your details.

You will receive an email asking you to click on a link. After this, you will receive a confirmation code via SMS on the mobile number you provided. This way, the system verifies that the data is correct. Afterwards, you can determine your own password.

Using the data just obtained (login name and password), you can now log in via the patient portal. Again, you will receive SMS verification. This is a legal requirement as we work with medical data.

Our WebApp

Through our WebApp, you always have access to your own medical data: view blood results and specialist letters, read what your GP has advised you. In addition, you can order repeat medication, schedule an appointment or ask for an e-mail consultation.

We are open for new registrations. Please fill in with form completely. You will receive an email confirmation from our practice within 72 hours. 

Do you have any further questions? Then you can make a call-back appointment by phone.

Survey results

Through My Medical Record, it is possible to access your medical record. It is possible to gain insight into the items below:

  • Laboratory results (summary of measurements your healthcare provider has shared with you)
  • Physical examination (the diagnostic values: blood pressure, height and weight)
  • Access log (overview of who has access to your medical records)
  • Report (For each topic, the preliminary conclusion and agreements made (E&P rules)
  • Hypersensitivities and peculiarities
  • My documents (incoming and outgoing correspondence, e.g. referral letters)
  • Current medication overview

E-Consult

For less urgent complaints, you can send us an e-Consult. Our practice then usually answers your query within 48h. Since you do not call, this saves us incoming calls and helps us spread out crowds.

Would you rather do it by phone?

Repeat medication

You can use this website to repeat your own chronically active medication. If you take one or more medicines for a longer period, the pharmacy has a free service especially for you: they make sure your medicines are ready at a pre-arranged time. 

File access

Your GP puts all the information about your health into a computer. This is called a medical file. For example, the GP puts in what diseases you have, what tests and treatments you have had and what medicines you are taking. This way, your medical information is all together.

Data exchange

How does the National Switch Point work?

Your (GP) doctor and pharmacies can connect their computers to the National Switch Point (LSP). Through the LSP, they can share your most important medical data. Another healthcare provider can view those medical details. The video below briefly explains how the LSP works when you come to the GP surgery.
